Membership Application Form

Membership Fees:
	Individual - $100.00
	Trainee (Student or Postdoctoral) - $25.00
	Corporate - Please contact CSPS at 780-492-0950 for more information.
At present, you may pay for membership by calling or faxing the office with your credit card number or mailing a cheque.
	Membership renewals:  An invoice will be emailed to you when it is time to renew your annual membership.  Renewals can be paid online.
